Information collected through forms
The quote form requests facility name and type, work email, phone, destination state or jurisdiction, purchase timeline, product specifications and volumes, optional supplier and quote information, preferred contact method and procurement consent. Quote-file uploads are unavailable. Use the text fields for non-sensitive business quote details.
Do not submit patient records, prescriptions, payment details, passwords or other sensitive information. This is a business procurement inquiry, not a clinical intake form.
Quote handling
Quote requests are sent to a service hosted on Netlify. The service uses Close CRM to record non-paid inquiries and follow-up tasks, and to send internal notifications and confirmation emails. Requests identified as paid traffic do not create a new lead record through this function; notification and confirmation emails still use Close. Those emails can contain submitted business information.
Your selected contact method and procurement consent are included with the inquiry for follow-up.
Analytics and attribution
The main page loads Google Analytics. Quote forms record campaign parameters and the landing-page address. The quote service checks advertising click identifiers supplied with a request to distinguish paid traffic.
Newsletter and applications
Newsletter and job-application forms use Netlify form handling. Information entered into those forms is submitted to Netlify.
